The Transitional Community Opportunities for Recovery and Engagement  Program (TCORE) is a collaboration between Human Resources Consultants, Inc. (HRC) and Transitional Living & Community Support, Inc. (TLCS) to provide intensive services to those adults living in Sacramento County who have a mental health diagnosis and are currently unlinked to any outpatient services.

•  Family Support Services
•  Advocacy
•  Skills Building Groups
•  Support with Employment
•  Support Enrolling in School
•  Transportation Assistance
•  Assistance applying for benefits (SSI/SSDI, SDI, Medi-Cal , GA , TANF, etc.)
