TLCS offers several programs that provide supportive services to enable individuals with a psychiatric disability living in a variety of housing programs to achieve the goals they have set for themselves. The following is a brief description of each of these programs.

MICA
The Mentally Ill Chemical Abuser (MICA) program offers intensive case management, support and educational groups, and substance abuse recovery support to individuals who are experiencing homelessness and have a psychiatric disability which is co-occurring with substance abuse. Single individuals initially live in the Homeless Transitional Apartment Program (see below). Upon discharge from the Transitional Apartment Program they may continue to receive these services for an additional 6 months. Individuals with dependent children are provided with rent subsidies to enable them to participate in the program. The staff also includes a psychiatrist and a nurse to provide quick access to mental health services.

Hope Team
The Hope Team provides mental health services, supported housing, intensive case management services, substance abuse recovery support and life skills training for adults who have a psychiatric disability and are experiencing homelessness or are at risk of homelessness. These services are aimed at improving quality of life and preventing the individuals from repeatedly cycling through the system. The staff also includes a psychiatrist and a nurse to provide quick access to mental health services and this program works closely with The Guest House, a division of El Hogar.

WORK
The Widening Opportunities for Rehabilitation and Knowledge (WORK) program provides mental health services, supported housing, intensive case management services, vocational services, substance abuse recovery support and life skills training services to adults who have a psychiatric disability, have been homeless and are ready to re-enter the workforce, or are in need of additional support to maintain established employment. Participants receive a rent subsidy and assistance to find the housing of their choice. The staff also includes a psychiatrist and a nurse to provide quick access to mental health services.

PACT
The People Achieving Change Together (PACT) program provides mental health services, supported housing, intensive case management services, substance abuse recovery support and life skills training services to adults who have a psychiatric disability and are experiencing homelessness. These services are aimed at improving their quality of life and preventing the individuals from repeatedly cycling through the system. The staff also includes a psychiatrist and a nurse to provide quick access to mental health services.
 

Drop-in services include access to computers, telephones and a food closet.

 
Transitional Community Opportunities for Recovery & Engagement (TCORE)

TCORE is a collaboration program between Human Resources Consultants, Inc, (HRC) and Transitional Living & Community Support, Inc. (TLCS) to provide intensive services to those adults living in Sacramento County who have a mental health diagnosis and are currently unlinked to any outpatient services.  TCORE provides recovery based services to all participants with intensive short-term services to ensure that their needs are met while in the process of connecting to ongoing outpatient mental health services that meet the individual and cultural needs of the participant. The length of services in the TCORE program may vary based on individual needs.  Each participant will complete an individualized goal plan that will assist in determining length of stay, however the length of service should not exceed 180 days and most participants will be linked to outpatient services within 120 days.

TCORE staff includes a Program Director, Clinical Director, Psychiatrist, and a Registered Nurse.  New participants are assigned to a Rehabilitation Team which includes a Team Leader, 4 Personal Service Coordinators, and a Consumer/Family Advocate.  TCORE also has a Benefits/Resource Specialist available to all participants.
